WEIGHT MANAGEMENT

The following listed conditions, and any medical condition the provider feels may risk a complication that outweighs benefit of treatment, are contraindications to treatment and will not be permitted medication therapy: currently pregnant or breastfeeding, type 1 or 2 Diabetes, chronic kidney disease, uncontrolled hypertension, hypersensitivity to Semaglutide/Tirzepatide or any component of the formulation, active alcoholism or drug addiction, suspected or diagnosed eating disorders, history of pancreatitis or cholecystitis, history of gastric bypass, sleeve or surgery, current weight loss therapy with another oral or injectable medication 

Adverse Reactions:

I understand and accept the most likely adverse reactions / side effects of Semaglutide/Tirzepatide treatment, and all GLP-1 products may include, but are not limited to: Nausea, diarrhea, vomiting, indigestion, xerostomia, abdominal pain, fatigue, dizziness and headache. Most clients experience some level of these side effects, particularly with initiation of medication therapy and with increases in dosages, however, most clients report these side effects as minimal / tolerable.  

Serious adverse reactions are very rare, but may include: Acute pancreatitis, cholecystitis, injection site infection, intestinal obstruction, acute interstitial nephritis / acute kidney injury (AKI), diabetic retinopathy, medullary thyroid carcinoma, angioedema and anaphylaxis. There is a black box warning for clients with personal or family history of medullary thyroid cancer or multiple endocrine neoplasia syndrome type 2 - this is due to studies performed on rodents and not clinically shown in human trials, however clients with these personal or family conditions will not be permitted to proceed with medication therapy.
